Administrative teams are a hidden asset to ensuring efficient and effective patient care. At the department for Stroke Rehabilitation, teams are well aware of the challenges patients face when it comes to receiving care from multiple providers or hospitals.

"Stroke patients are often treated for multiple concerns, and require file transfer between providers. It's easy for mistakes to be made, and this can mean appointments need to be rescheduled without notice in advance." says Pauleen Jam, Team Lead.

"At UHN, we are fortunate to have many experts in care, who patients travel to see. This means coordinating care with external providers they see closer to home. Not every office has the resources to follow up on the success of file transfer."

Partnering with Toronto-based digital healthcare platform PocketHealth, the team worked with UHN Digital to integrate access to Pocket Health for UHN patients through the myUHN Platform.

MyUHN is a digital platform where patients and healthcare providers can securely access files and manage appointments. The administrative team suggested the addition of a link to PocketHealth, where patients can use the same login to access external healthcare files that may not have reached the UHN team.

"Just 3 months after rolling this out we have seen a significant reduction in the need to reschedule appointments because the doctor doesn't have all of the files they need. Patients can stop by our desk before their appointment and share any files our team might be missing."

Saving time for both healthcare providers and patients, this administrative team at Toronto Rehabilitation Institute shows how innovative partnerships across the healthcare system contribute to our shared vision of A Healthier World.
